Bilateral total revision surgery of the hip joint is a procedure to replace the previously implanted prostheses of both hip joints with new ones. This is often necessary due to complications or failure of the original implants, such as loosening, wear, dislocation, or infection. The operation involves removing old components, restoring bone tissue or its damage, and installing new implants. Revision surgery is more complex than initial hip replacement surgery because it removes existing hardware and addresses any issues with bone loss, tissue damage, or infection that may have occurred since the initial implantation. Bilateral total hip revision usually takes 3 to 4 hours and is performed through a 10-20 cm skin incision on both hips. We use SMITH & NEPHEW prostheses because their materials are certified and recognized worldwide. In addition, these prostheses are durable and have a low risk of rejection. An important aspect is the smoothness of movements and the possibility of postponing joint replacement operations for a more extended period than in conventional standard prostheses. To preserve the integrity of the nervous system during total hip arthroplasty, "neuromonitoring" is used, which provides the surgeon with information about nerve conduction and significantly contributes to the preservation of the integrity of the nervous tissue. In some cases, patients undergoing hip revision surgery may already have an infection associated with their existing implants. Antibiotic treatment is essential to control and eradicate the infection before the new implant is inserted. Failure to treat existing infections adequately can lead to complications such as implant loosening, bone loss, and systemic bacterial disease. Even if there is no existing infection, antibiotic prophylaxis is often administered before surgery to reduce the risk of postoperative infection further. This is particularly important in procedures involving the insertion of prosthetic implants, where the consequences of infection can be severe. Patients usually start walking with support (walker, cane with a bracelet) on the day of surgery or the next day. You can sit, stand, and go up and down stairs with support. Patients who can perform these movements without difficulty and with balance can leave the hospital and go home if their overall health is adequate. Two-sided total hip revision surgery offers comprehensive correction by simultaneously addressing issues in both hips, reducing the need for multiple surgeries and potential complications associated with repeated procedures. It ensures symmetrical hip function, balance, and alignment, promoting more uniform recovery and enhanced overall mobility. Additionally, it may lead to shorter overall recovery times and reduced healthcare costs compared to undergoing separate surgeries for each hip.
