Pressurized intraperitoneal aerosol chemotherapy (PIPAC) is a way to treat cancer by putting chemotherapy drugs directly into the peritoneal cavity, which is the space in the abdomen where the organs are. A laparoscope is used to do PIPAC. It is a thin, lit tube with a camera on the end that is put into the abdomen through a small cut. The laparoscope is connected to a machine that turns the chemotherapy drugs into a pressurized spray sprayed all over the peritoneal cavity. Cancers that spread to the peritoneal cavity, like ovarian or stomach cancer, are usually treated with PIPAC. It can be an excellent way to treat advanced cancer patients who aren't getting better with other treatments or can't have surgery because of other health problems. PIPAC is a novel procedure that allows surgeons to treat the abdominal cavity with chemotherapy in aerosol (gas) form. This procedure is performed through a minimally invasive laparoscopic approach. PIPAC treatment can often be repeated several times to maximize the anti-cancer effect. PIPAC is usually indicted in patients who are not candidates for the CRS HIPEC procedure because their tumor cannot be removed surgically. One of the benefits of PIPAC is that it can deliver high doses of chemotherapy directly to the cancer site. It may be more effective at killing cancer cells than chemotherapy delivered through the bloodstream. PIPAC may also have fewer side effects than other types of chemotherapy because the drugs go straight to the peritoneal cavity and don't travel through the rest of the body. More research is needed to determine PIPAC's long-term effectiveness and safety.

How is PIPAC different from HIPEC?

The indications and techniques for the PIPAC technique are entirely different from CRS HIPEC. PIPAC is generally recommended in patients who are not candidates for CRS HIPEC. PIPAC patients are treated with intraperitoneal chemotherapy, but the tumor deposits are not surgically removed. During PIPAC, the chemotherapy drug is administered into the abdominal cavity as an aerosol, in contrast to HIPEC, when the drug is administered in the form of a liquid.

What are the indications for using PIPAC?

PIPAC is less toxic than HIPEC and is used when cytoreductive surgery with HIPEC cannot be performed due to disease progression or the patient's condition.
