YOUNIFY Clinic is a physician-led integrative medicine and wellness center in the heart of Silom, Bangkok, Thailand. We bring together evidence-based, minimally invasive care across eight service lines, from advanced weight management and joint care to vision, pain medicine, and surgery — under one roof. Our approach is built on three principles: care is physician-directed, decisions are grounded in current medical evidence, and, wherever clinically appropriate, we favor minimally invasive options that help most patients return to daily life quickly. Every treatment plan is individualized, and outcomes are always discussed honestly, including what a given procedure can and cannot do. The clinic was founded by Dr. Noppachai Siranart, whose background includes postdoctoral research at Brigham & Women’s Hospital / Harvard Medical School and Mayo Clinic. Under his direction, our medical team combines international training with a calm, unhurried patient experience designed for both local and international patients. In general, we are an outpatient facility, although some of our procedures require inpatient hospitalization and use of a surgical room. Therefore, we work in close cooperation and rent an operation room at our JCI/AACI-accredited hospital partners to ensure maximum safety and optimal results for our patients. Our treatment spectrum is defined by eight clinical service lines: 1) weight management & metabolic health (endoscopic sleeve gastroplasty (ESG), intragastric balloon, physician-supervised GLP-1 programs, metabolic and liver assessment); 2) joint & pain medicine (genicular artery embolization (GAE) for knee osteoarthritis, transarterial microembolization (TAME) for chronic tendon pain, image-guided neck and back procedures by pain medicine physicians); 3) aesthetic & structural surgery (face and neck lift, rhinoplasty, blepharoplasty, breast surgery, tummy tuck, lipo 360, body contouring); 4) women's health & healthy aging (hormonal and life-stage care, bone health and fracture-risk assessment, longevity and preventive planning); 5) aesthetic dermatology & skin (botulinum toxin, dermal fillers, laser treatments, skin boosters and rejuvenation programmes); 6) health screening & GI (executive health screening, gastroscopy and colonoscopy under sedation, fatty liver (MASLD) assessment); 7) vision care (LASIK, ReLEx SMILE, ICL, cataract surgery with premium lens replacement, eyelid procedures); 8) precision oncology (CT and MRI, tissue NGS and liquid biopsy, molecular tumor board review, targeted therapy and immunotherapy). Our strongest differentiators for international partners are endoscopic sleeve gastroplasty and genicular artery embolization: two minimally invasive, non-surgical pathways with high patient demand, travel-friendly recovery, and very few competing providers in this region. Our core medical service is detailed and defined as follows:Weight Management - Endoscopic Sleeve Gastroplasty & the Non-Surgical Ladder For many people carrying excess weight, the hardest barrier is not motivation; it is the fear of surgery. Endoscopic sleeve gastroplasty, often described to patients simply as “stomach tightening,” offers a path to meaningful weight loss without any surgical incision. The stomach is reshaped from the inside using an endoscope passed through the mouth, so there are no cuts on the abdomen and no external scars. With ESG, patients typically lose 15-20% of their body weight within one year, though results vary by individual and depend on the lifestyle support that follows the procedure. Just as important as the number on the scale is what that weight loss can do for overall health: as excess weight comes down, many patients see improvement in weight-related conditions such as type 2 diabetes, high blood pressure, high cholesterol, fatty liver, and sleep apnea. These changes vary from person to person and are monitored by our medical team over time. ESG is performed endoscopically under sedation. Using a suturing device passed through the mouth, our physician places a series of internal sutures that reduce the stomach's working volume and slow its emptying rate. This helps patients feel full sooner and eat smaller portions. Because there is no abdominal surgery, most patients go home the same day and recover quickly, returning to normal activities within a short period. As with any medical procedure, some temporary discomfort, such as mild nausea or cramping in the first few days, can occur and is managed by our team. A full ladder of options for patients who do not want surgery. Not every patient is the same, and not everyone needs or wants a procedure. Surgery is carried out under appropriate anesthesia in a controlled surgical environment, using techniques and instruments that meet international standards. Recovery time depends on the procedure and the individual; our team provides a clear pre- and post-operative plan and follow-up care to support healing at every stage. Because these are surgical procedures, they carry risks that are discussed openly during consultation, and results develop gradually as swelling settles. Knee Osteoarthritis - Genicular Artery Embolization (GAE) & Knee Replacement Knee osteoarthritis rarely has a single right answer. YOUNIFY Clinic proposes a complete, staged pathway so patients can be matched to the least invasive option that fits their condition and step up to surgery only when it is truly indicated. Genicular artery embolization (GAE) is a minimally invasive, needle-based treatment for knee pain associated with osteoarthritis. In an arthritic knee, abnormal new blood vessels form around the joint, contributing to inflammation and pain. In GAE, our physician uses image guidance to reach these tiny vessels via a small catheter and reduce their abnormal blood supply, calming inflammation at its source. There is no surgical incision, no implant, and most patients go home the same day. For suitable candidates, results may last several years in some cases (typically 1-5 years), depending on the individual. GAE is often an appealing option for patients whose pain is not controlled by medication or injections but who are not yet ready for, or wish to delay, joint replacement. When osteoarthritis is advanced and the joint surface is severely worn, total knee replacement (arthroplasty) remains the definitive, well-established treatment. YOUNIFY coordinates surgical care with our partner surgical team, guiding international patients through assessment, the procedure, and structured rehabilitation. For the right patient, replacement can restore mobility and substantially reduce long-standing pain. From injections and physician-guided rehabilitation, to GAE, to knee replacement, patients move along a single pathway under coordinated medical guidance, so the choice is always matched to the stage of disease, not to what a clinic happens to offer. Chronic Tendon & Joint Pain - Trans-Arterial Micro-Embolization (TAME) Some of the most frustrating pain conditions are the ones that simply will not settle: plantar fasciitis in the heel, a frozen shoulder that limits every reach, and tennis elbow that flares with daily tasks. These conditions share a common feature: abnormal new blood vessels and nerve growth that keep the area inflamed and painful long after the original injury. Transarterial microembolization, from the same minimally invasive family as GAE, targets exactly this. Using image guidance, our physician reaches the abnormal microvessels feeding the painful area through a fine catheter and gently reduces their blood supply, helping to break the cycle of chronic inflammation. It is a needle-based, day procedure with no surgical incision. TAME may be considered for chronic, treatment-resistant conditions, including:Plantar fasciitis - persistent heel and arch pain;Frozen shoulder (adhesive capsulitis) - pain and stiffness that limit shoulder movement;Tennis elbow (lateral epicondylitis) - pain over the outer elbow that flares with gripping and lifting. TAME is typically considered for patients whose pain has persisted despite conservative care such as rest, physiotherapy, medication, or injections, and who prefer to avoid surgery. Suitability is confirmed after clinical assessment and appropriate imaging. Most patients return home the same day. Vision Care - LASIK, ReLEx SMILE & Cataract Surgery Good vision shapes daily life, and YOUNIFY’s Vision Care service offers a range of modern options to correct refractive error and treat cataracts, using laser and surgical platforms that meet international standards. LASIK - a widely used laser vision correction procedure that reshapes the cornea to reduce dependence on glasses or contact lenses for suitable candidates;ReLEx SMILE - a minimally invasive, flapless laser vision correction technique performed through a small corneal opening, offered for eligible patients seeking a gentle, keyhole approach;Cataract surgery - removal of the clouded natural lens and replacement with a clear artificial lens to restore vision affected by cataracts, a common part of aging;ICL (implantable contact lens) - an additional lens-based option that may be considered for patients who are not ideal candidates for laser correction. Every vision patient begins with a thorough eye examination to determine which procedure, if any, is suitable, as candidacy depends on factors such as corneal thickness, prescription, and eye health. Laser vision correction is typically a quick, outpatient procedure with a short recovery, while cataract surgery is a well-established day procedure. Our team explains the options, benefits, and limitations so each patient can make an informed choice. Pain Medicine - Neck & Back Pain Persistent neck and back pain can quietly narrow a person’s whole life. YOUNIFY’s pain medicine service offers a structured, minimally invasive approach delivered by pain medicine physicians, with the goal of first identifying the true source of pain, then treating it precisely. Every procedure below is image-guided, needle- only, and performed as a same-day outpatient visit, with no surgical incision. Our interventional pain options include:Diagnostic medial branch block - a precise test injection used to confirm whether the small facet joints of the spine are the source of pain, guiding the next step of treatment;Medial branch radiofrequency (RF) ablation - using controlled radiofrequency energy to calm the specific nerves carrying pain from the facet joints, often after a positive diagnostic block;Facet joint injection - targeted injection into the facet joint, available with steroid, PRP, or prolotherapy depending on the clinical picture;Cervical epidural steroid injection - delivering anti-inflammatory medication to an irritated nerve in the neck;Selective cervical nerve root injection - a focused injection at a specific nerve root, both to relieve pain and to help pinpoint its source;Trigger point injection - treating tight, painful muscle knots that contribute to neck and back pain. Because diagnostic and therapeutic tools are used together, care is tailored to each patient rather than applied as a single generic injection. Many patients use these procedures to reduce pain, improve function, and, where possible, avoid or postpone spine surgery. Suitability is determined after clinical assessment and imaging. Laparoscopic & Robotic Surgery - Hernia, Sleeve Gastrectomy & Whipple For conditions that require an operation, minimally invasive surgery can mean smaller incisions, less blood loss, and, for many patients, a faster, more comfortable recovery than traditional open surgery. YOUNIFY Clinic coordinates laparoscopic (keyhole) and robotic-assisted surgery across a range of procedures, from routine to highly complex, delivered by our surgical team in a fully equipped setting. Procedures include:Hernia repair - laparoscopic and robotic techniques for inguinal, umbilical, and other hernias, typically with small incisions and a quicker return to activity;Sleeve gastrectomy (SG) - a well-established bariatric operation that reduces the size of the stomach to support significant, durable weight loss for eligible patients, performed minimally invasively;Whipple procedure (pancreaticoduodenectomy) - a major, complex operation for tumors and conditions of the pancreas, bile duct, and duodenum, where robotic and laparoscopic approaches may be used to reduce surgical trauma in appropriate cases. Robotic assistance gives the surgical team enhanced precision, magnified 3D visualization, and fine instrument control, particularly valuable in delicate, complex operations. As with all surgery, each case is individually assessed, risks and benefits are discussed openly, and the choice of approach is made on clinical grounds. Recovery varies by procedure and by patient, and our team provides structured pre- and post-operative care throughout. Precision Oncology - Diagnostic Imaging, Genomic Profiling & Targeted Therapy Cancer care has changed. Two patients who receive the same diagnosis on paper may have very different tumors at the molecular level and may therefore respond very differently to the same medication. YOUNIFY’s precision oncology service is built around that reality: we closely examine what drives each individual tumor and then match treatment to those findings whenever a suitable option exists. Every step is delivered in-house by our own medical team, from the first scan through genomic testing and molecular tumor board discussion to treatment and follow-up, so patients are not passed between unconnected providers at a difficult time. Step 1 - Diagnostic imaging CT and MRI: Accurate imaging underpins every decision that follows. Our diagnostic service uses computed tomography (CT) and magnetic resonance imaging (MRI) to help detect and characterize tumors, define their extent and their relationship to nearby structures, support staging, and monitor how disease behaves over time. Imaging is reviewed together with laboratory and pathology findings rather than in isolation, and follow-up scans are scheduled at intervals appropriate to each patient’s diagnosis and treatment, so that change can be identified early and the plan adjusted. Step 2 - Genomic profiling: tissue NGS and liquid biopsy: Molecular profiling is what turns a diagnosis into a treatment map. YOUNIFY offers two complementary approaches:Comprehensive tissue genomic profiling (NGS) - next-generation sequencing of a tumor tissue sample to look for genetic alterations and biomarkers that may be addressable with targeted or immune-based therapy;Liquid biopsy (circulating tumor DNA) is a blood-based test that can be used when tissue is limited or difficult to obtain, and to follow molecular changes over the course of treatment. Testing may identify alterations in pathways such as EGFR, ALK, ROS1, HER2, BRAF, KRAS, RET, NTRK, and MET, as well as immune-related markers such as PD-L1 expression, microsatellite instability (MSI), and tumor mutational burden (TMB). We are always clear with patients on one point: not every tumor carries an alteration for which a matched treatment is available, and when no such alteration is found, our team openly discusses the remaining evidence-based options. Step 3 - Molecular Tumor Board (MTB): A genomic report is only useful when placed in clinical context. Every profiled case is discussed at our molecular tumor board, a structured, multidisciplinary review that considers imaging, pathology, molecular findings, prior treatment, and the patient’s overall condition together. The board weighs the strength of the evidence behind each possible option and produces a written recommendation for the treating physician, together with alternatives should the first approach prove unsuitable. This is the step that keeps precision oncology honest, and it is why a test result never becomes a treatment decision on its own. Step 4 - Targeted therapy, oral TKIs, and immunotherapy: Where profiling identifies a suitable target, treatment is matched to it. Our precision oncology treatment options include:Targeted therapy - medication directed at the specific molecular pathway driving a tumor, rather than at dividing cells in general;Oral tyrosine kinase inhibitors (TKIs) - targeted medication taken by mouth which, for suitable patients, can allow treatment to continue at home between scheduled clinic reviews, without repeated infusion visits;Immunotherapy is a biomarker-guided treatment that works by supporting the patient’s own immune response against the tumor and is considered where markers such as PD-L1, MSI, or TMB suggest it may be appropriate. Patients on treatment are monitored with scheduled clinical reviews, laboratory testing, imaging, and, where useful, repeat liquid biopsies, so that tolerability and response can be assessed and the plan revised as needed. Side effects are anticipated and managed actively rather than reactively. Precision oncology is not appropriate for every patient or every tumor, and the reasonably expected benefit of any option is discussed before treatment begins. For international patients, our team coordinates the sequence: imaging, sampling, genomic testing, molecular tumor review discussion, and the start of treatment, so that time in Bangkok is used efficiently rather than spent waiting. Reports are provided in English, oral treatment can often be continued at home under scheduled review, and follow-up can be arranged in coordination with the patient’s physician in their home country.
